Health and Safety Report Form

 

This is a generic form that you could use as a company or school to record an accident or incident that has happened or could have happened in the workplace. Note that this form is quite extensive and you could remove or leave parts unfilled if they are not relevant.

Job Safety Analysis

 

This is a form that could be filled for a specific event or gig you may be involved in. It goes through the key tasks involved in the set up, performance and pack down. Then analyze the hazards, risks and ways these can be controlled. It also includes information like an emergency plan and emergency contacts.
